1321647187 2011-11-18 Inspection revealed that landowner has built a fence and gate across lease road blocking well owner from access to well site and also hindering well owner from resolving Compliance Notice issued for leaking tank. I had requested the well owner to remove damaged tank from site, replace it with a new tank and properly identify it. They must also report production as required by law.
1323734323 2011-12-12 My inspection revealed that the well owner has blocked access to the well. Removal of the tank from the site, as previously requested to comply with notice issued on 11/2/2011, cannot be completed. Someone has recently posted a sign at entrance indicating that the well is toxic. It was noted on this inspection that the tubing pressure was at 240 PSI as compared to 35 PSI on an earlier inspection which is the sales line pressure. The well valve that was open before is now closed. I contacted the owner and he stated that he did not close valve.

1387500249 2012-01-17 Inspection revealed that there has been no change in status of well. It was noted that well has 900 PSI on it and all valves are still shut at wellhead. The old storage tank still needs to be removed from site once the well owner and the landowner come to agreement to remove gate from existing lease road. Note: The DNR North Region Supervisor and DNR Hydrogeologist accompanied me during inspection.
1427302357 2012-10-04 Inspection revealed that there is no change in status of well and owner is waiting on legal system to get lease issues resolved before entering property.
1788016617 2011-12-02 Inspection this day at the request of Columbus DNR regarding complaint by landowner, Jeff Bond. I found this casing well with tubing and frac valve installed. All but one valve were closed. One valve appeared to be open slightly. Proper identification posted at the well. The production tank has been disconnected, emptied and ready remove. There is no meter in the horizontal separator. According to landowner, he has issues with possible lead paint on tank. I explained that we did not regulate that. Columbus DNR had mentioned that the landowner had tested his water well with a personal test kit and found high levels of mercury and lead. I did not find any well related issues that would cause groundwater impact of this type and the landowner did not bring up any ground water issues during our meeting. He also complained about the new lease road which has washed out at the creek crossing. He has locked wellowner/producer out. His main complaint this day was about gas being co-mingled and metered with another landowner's gas and how they can calculate the correct amount of royalties to issue. I told him that was an issue he needed to have attorney address. To date, there has been no production turned in for this well but Miley has been sending him small checks, which he cashes. I believe I satisfactorily answered most of his questions and issues but he is still not satisfied about the co-mingling of gas. He also believes this well is too close to the lease line (landowner measurement of 77') but he could not show me where property lines were. DNR Inspector Dave Ball has inspected and wrote a Notice of Violation on this site which has not been resolved yet. Pictures were taken and it was recommended that he contact an attorney to resolve leasing issues.
293833606 2013-08-13 I received a call from Mike Sherron with the Ohio EPA, he asked if I could meet him and inspect a compliant on Mudd Run Road. I met Mr. Sherron at Mudd Run. We inspected the area and found no signs of illegal dumping as alledged. We were able to determine that the substance that was applied to the road was a dust suppressant.
373716562 2012-03-20 Inspected well site along with supervisor too determine status of outstanding compliance notice, owner and landowner are in legal litigation over lease rights and access to wellhead and tank site. Suggested to well owner to have attorney to get proper documents filed with local court system to allow owner to enter property to resolve erosion and safety issues pertaining to compliance notice.

Note: Tubing pressure at 950 psi during our inspection, and landowner has installed gate at the end of lease road it was unlocked on the day of our inspection.

I told him to replace the production tank, to properly construct the containment dike after replacing the tank and also report production on well as required by law. Well is equipped with 8" surface casing, 4.5" production casing and 2" tubing.
