Took photos and GPS of the well.

840379981 2011-03-03 Well lacked proper identification. Oil had escaped near a 1/4 " ball valve and had contaminated a 4x4 foot area around well head. The Id at the common tank battery was correct. 50 barrel production tank had leaked and dike had allowed a small area to become contaminated with brine. 1501:9-9-05 (A)(10) All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tanks as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tanks each well-head shall be identified as to owner, permit number where available, well number and lease name. Area around well should be cleaned. All oily materials shall be soaked up and disposed of properly. Brine contaminated area around dike shall be soaked up and disposed of properly. Dike shall be reworked to no longer allow contaminates to seep out. 50 barrel tank shall be replaced or repaired. All contaminated soils shall be removed and ground shall be mulched and reseeded. 9-1-07
