-2061027469 2011-11-14 The pits are closed, vegetative cover established and there is no drilling equipment on site. Two areas (45' x 4' and 30' x 15'), on either side of road, had lost soil due to erosion. An unused pit liner remains on site, by the tank battery dike. All bare areas shall be seeded and mulched in order to encourage vegetative cover and all unused pit liner shall be removed. NONE5
-333428 2010-12-20 Surface cement job. TD 644', 8 3/4" hole. Ran 637' of 7" surface casing (20lb). Spud date 12-15-10. Drilled on fluid. No conductor set. Encountered red rock at 400' and 580'. Did not encounter water during drilling. Frac tank with fresh water had frozen valve that was broken and had to cut ice through top access for water. Pump truck pop off blew up due to valve on casing being closed. Bulk truck busted their hydraulic line and was not able to be repaired in the field. Another bulk truck was sent to location took approximately 3 hours to arrive on location. Pumped gel and LCM to condition the hole prior to pumping cement. Good circulation of gel and fluids. No cement returns to surface (figured 40% excess). Ran 1.5" plastic tubing down to 25' and grouted to surface. Cement stayed at surface then settled to 6' from surface and stayed.
-474664594 2012-01-03 Erosion control is in place and all areas non-essential to production have adequate vegetative cover. The lease road has been graded and water bars have been installed. Preliminary Restoration pass.
-618127744 2010-11-19 Rainbow Drilling has rigged up, they have not spudded yet. Pit has been constructed and lined. Pit is full of fluid for drilling on mud. Three frac tanks are on location for water storage.
-678669985 2011-08-29 Checking on restorations. Pit closure only thing required yet, but reclamation will be due soon. Pits closed and well has good ID, but no seed/mulch and no grade. Production equipment still remains on site.
-678669985 2011-08-29 Preliminary Restoration inspection. Pits have been closed and well has the proper identification posted. The location has not been graded or seeded/mulched. Some drilling equipment still remains on site.
-826307496 2011-02-04 4.5" long string cement job. TD 1989'. Ran 1980' of used 11 lb 4.5" casing, in a 6.25" hole. Circulated fluids to surface prior to pumping cement. Accompanied by Josh Garland MRI 1 and Andy Thomas MRI 1.
1310065334 2011-07-05 AD test. Test at 370 lbs with Nitrogen. Zero loss in 60 minutes.
334419357 2010-12-21 Received phone call from Jeff Burke (Rainbow drilling). Jeff told me that he topped off the cement with bag mix. He said that the surface cement was at 6' from the surface.
853750509 2011-09-07 Preliminary Restoration check. The pits have been filled, the site has been graded, seeded/mulched but no vegetative cover established yet. The proper identification is posted at the well. There is no erosion control at the site, however there is no erosion noted.
