| 1268829225 | 2010-03-05 | Domestic well status. No identification. Well has a 5 1/2" steel by 2" swedge that is half out of the ground. A 2" Sch 40 PVC female adapter is attached to the swedge. A 5' piece of PVC is glued into the female adapter with a 2" coupling and a 2" by 1" bushing glued on other end of pipe. A 1" male adapter with a brass ball valve attached on top. No lines or production equipment. I talked to the new landowner Shane Clark. Shane told me that the well was sold to him along with the property. He also told me that before he purchased the property someone hit the well with a brush hog and damaged the tubing. Shane installed the 2" PVC on the well. I told him that must be changed due to the potential hazard that pressurized PVC represents. Pictures were taken of the well. I also explained the requirement that he must be a registered well owner. Shane provided me with his address and phone number. I told him that I will send him a packet and well records for his use. I also told him that after he is registered he will need to place Identification at the well. ID specs will be included in letter with domestic well packet. | ||||

According to 1501:9-9-05(A)10/1509:23 well identification sign shall include owner's name, county, township, well name and number, permit number for the well, and emergency contact number. There is no tank battery associated with this well. According to Marvin Vanbuskirk, he owns the Dummitt property on which this well exists. However the well ownership was never transferred from Cletis Dummitt to Mr. Vanbuskirk. Cletis Dummitt is deceased according to Mr. Vanbuskirk. According to 1501:NONEH/1509:31 he failed to notify DOGRM of assignment or transfer of this well. This well shall be transferred to current owner/operator. Sometime around 2007, Mr. Vanbuskirk sold this well and the accompanying parcel of land this well is located on to Mr. Shane Clark, 3570 Millers Run- Back Run Road, Lucasville, Ohio 45648; phone number 740-820-3825 (no longer in use). At the time of my inspection, the six feet 2" Schedule 40 Polyvinyl Chloride, that was observed by Jon Scott in an earlier inspection attached to the 2" steel threaded production well tubing, was unattached and lying on the ground next to the well. My PhD6 gas meter detected methane gas at 0.55 meter reading units from the production tubing.. According to 1501:9-9-05(A)1/1509:20 the well is insufficiently equipped to prevent the escape of gas. The owner shall repair the faulty equipment on the well to correct the problem. PLEASE NOTIFY MR. VANBUSKIRK WHEN YOU ARE PLANNING TO TRAVERSE ON HIS PROPERTY AND INSPECT THE WELLS.
