-1463435573 2014-01-02 The well is in production and is equipped with 4.5", tubing, and rods, however there is no pumpjack. The 4.5" casing head is leaking gas. There is a separator and a 550 gallon plastic tank in the dike. The tank has a leaking nipple. I called Larry Berringer with Petroquest and he said he would get out there soon and take care of the issues. There is no posted identification. Operator needs to repair or replace all faulty equipment and cleanup any contamination within the dike. Operator must also post legible identification. 9-1-07

-1705929019 2014-06-19 The plastic tank and contamination has been removed from dike. The well is now in compliance.
-397553519 2014-04-29 Compliance Action Follow-up -1463435573: The tank is dripping at the bad nipple. Rainwater has filled up the tank some and is forcing some of the tank fluid out the seams. I talked to Larry Berringer of Petro Quest and he is going to check it out tomorrow. Once the ground dries out they will fix or get rid of the tank.
1448057810 2014-01-31 Compliance Action Follow-up -1463435573: Identification has been posted and the wellhead is no longer leaking gas. The plastic tank is still an issue however the fluid level is low to the point where it is not leaking at this time. There is still small amount of contamination inside the dike around the tank. I will continue to follow-up with Larry until well is in compliance.
