Water well driller was removing pump and surging well to develop, on 1/13/2011. It was determined that the water well had collapsed and was not related to the oil well drilling operation. Complaint was resolved per the landowner.

1347110319 2012-08-21 Inspected well for conversion to a Salt Water Injection Well (SWIW). Well is located in open field. About ten residential homes and farm houses with water wells, located within a 0.25 mile radius and the closest water well is 670 feet west of the injection well. The water wells range in depth from about 60 feet to 140 feet, but the deepest is 250 feet. The homes in the area are reported to have rural water supply. A small farm pond is located less than 300 feet to the north east of the well. If the tank battery is to be located near the road with an unloading pad then double lined injection lines should be warranted. Contacted Landowner, at time of inspection, unaware of the operators intent to convert well into injection well.
