At the time of inspection, no E&S or IW issues. Pit aligned with 20 mil plastic and security fence. No freeboard issue. US Energy is contracted. Conductor pipe (13 7/8﾿) set at 40 ft (sand). Water detected at 70 ft and 615 ft. Stratigraphy consisted of coal, ss and sh. Surface casing (8 7/8) set at 1574 ft. Universal present to cement surface casing. Pressure at 800 psi, 4.94 bbl flow rate, density at 8.36 dh consist throughout cementing. A total of 290 sks Class A cement was used. Plug down at 9:48 am. No signs of gas leak from around well and equipment.

1905465 2010-08-11 No Violations Noted At the time on inspection, site was properly signed with E&S plan available. Well drilled, not set for production. Filter fence installed down gradient from pad. One fenced drill pit located onsite appears structurally sound. Over 2ft of lined freeboard observed. Recommend that operator rock line ditchs surrounding pit and pad in order to reduce the potential or accelerated erosion during heavy rain events. Access road stable. No accelerated erosion or sedimentation or industrial waste issues were noted in relation to well site or access road at this time.
1950702 2011-02-04 No Violations Noted At the time of inspection, site Id sign and ES plan present. Well drilled and in production. Brine tank appears sound.

Observe smoking coming out of well production equipment, IMMEDIATELY called Mark Hopkins of Kriebel whom call the well tender. Mark later confirmed, well tender stated it was an over heated heater releasing steam and it was being addressed. Temporary bmp﾿s in place, site not yet permanently restored. No accelerated erosion or sedimentation issues noted with site or access road. Access road and site snow covered.

2026109 2011-12-13 No Violations Noted At the time of inspection, well drilled (spud 8/02/10 ;drilling completed 8/09/10), equipped, and in production. Well tag present. Well site permanetly restored with adequate vegetation and rock. No accelerated erosion or sedimentation issues noted. One brine tank, appears structurally sound, located onsite and surrounded by vegetative dike. Access road stable. All temporary bmp's have been removed. No violations noted at time of this inspection.
