Well tag present. Well head consisted of 8﾿ pipe reduced to 4﾿ to 2﾿ shut off valve. No Production equipment present and no production occurring at this well head. Inspector has requested that the Water Quality personnel inspect site for site restoration. As noted in the attached picture, surface water has created a pond around well. The area has not been graded sufficiently to allow surface drainage. At the time of inspection, no IW issue at the site.

1843773 2009-11-09 No Violations Noted At the time of inspection, the well was properly signed, but never put into production. Drilling commenced on 02/05/2008. Noted the well site was not adequately/uniformly restored, although an attempt was made, the well site lacked successful vegetative growth; in addition ponds of water have formed surrounding the well site due to insufficient grading. Per conversation with Brian Welsh of Admiral Bay on 11-17-09, the areas of erosion will be filled and regraded, as well as reseeded promptly. A follow up inspection will take place to ensure measures were taken to eliminate accelerated erosion and sedimentation concerns surrounding well site and access road.

2177720 2013-06-12 No Violations Noted Well plugged on 11/16/2012. Monument buried. Located between ag field and treeline. Access road in good condition. No accelerated erosion or sedimentation issues observed in relation to well site or access road.

Site has been regraded. Minimal vegetative growth observed. As per current regulations, Operator has until 8/16/2013 to achieve full restoration of well site.

Department recommends Operator re-seed/mulch site to promote vegetative growth and monitor and maintain site to ensure permanent stabilization and final restoration is achieved.

2198773 2013-07-03 In Compliance with Policy Department contacted Operator representative Kent Keppel via phone to discuss current status of site. Department informed Mr. Keppel that it appears site has been influenced by landowner activity (i.e. ag activities) and as per current regulations, Operator has until 8/16/2013 to achieve full restoration of well site.

Landowner requested that access road remains. Department recommended Operator submit documentation from landowner verifying that landowner is utilizing well site for agricultural activities and verifying landowner previously requested access road remains. Operator appears to be in compliance with current policy at this time.

UPDATE: 8/28/2013 Department received above referenced documentation from landowner via Operator representative Kent Keppel.

2204199 2013-09-05 No Violations Noted Well plugged on 11/16/2012. Buried monument. Located between ag field and treeline. Access road in good condition. No accelerated erosion or sedimentation issues observed in relation to well site or access road.

Site is permanently stabilized and adequately restored to pre-existing conditions.

As per landowner request, access road remains (See Attached).

Based on today's inspection, Department is recommending bond be released for said well.
