The area has not been graded sufficiently to allow surface drainage. At the time of inspection, no IW issue at the site.

Noted the well site was not adequately/uniformly restored, although an attempt was made, the well site lacked successful vegetative growth; in addition ponds of water have formed surrounding the well site due to insufficient grading. Per conversation with Brian Welsh of Admiral Bay on 11-17-09, the areas of erosion will be filled and regraded, as well as reseeded promptly. A follow up inspection will take place to ensure measures were taken to eliminate accelerated erosion and sedimentation concerns surrounding well site and access road. | |||

Department recommended Operator submit documentation from landowner verifying that landowner is utilizing well site for agricultural activities and verifying landowner previously requested access road remains.
