Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
2145106 2013-02-20 No Violations Noted CONDUCTOR SET WAITING FOR TOP HOLE RIG TO COMMENCE DRILLING.
2152978 2013-02-28 Pending The Department was at the above referenced site to conduct a routine/partial inspection. The Department observed 6 wells spudded on site. The system says the wells were spudded on 2/11/13. There was no pre operational inspection and disturbed areas are not temporarily stabilized. There was a slide on the site.
2152986 2013-03-28 Violation(s) Noted 664101 102.22 - Failure to achieve permanent stabilization of earth disturbance activity.
2152986 2013-03-28 Violation(s) Noted 664102 102.4 - Failure to minimize accelerated erosion, implement E&S plan, maintain E&S controls. Failure to stabilize site until total site restoration under OGA Sec 206(c)(d)
2164579 2013-04-17 No Violations Noted The Department was at the above referenced Marcellus Well to conduct a Compliance Evaluation Inspection. The Operator was on site during this inspection. The Operator is currently drilling on site. The Department discussed the secondary containment violations and will remove the violations from the NOV. The Operator has excavated some of the hillside where the slip occurred and placed rock to stabilize the hill. The Department and the Operator discussed fixing the slide on site. The Operator will have to get a modification to the E/S Plan before fixing the slide.
2184799 2013-07-22 In Compliance with Schedule This inspection report is to rescind the secondary containment violation from 3/28/13. The only violation for that day will be in regards to the E/S and/or stabilization issues. To resolve those violations, you will probably have to apply for a major modification to the ESCGP-1. The area of concern is outside the permit boundaries and/or limit of disturbance. Please contact the regional Engineers in Pittsburgh or myself if you have any questions.
