2199731 2013-08-01 No Violations Noted On August 1, 2013 I stopped at EQTï¾’s Gallagher site for a perimeter BMP inspection; I met with Rex Nelson the Construction Supervisor.

I found operating permits for five wells: 592057 (125-27027) 592052 (125-27022) 592056 (125-27026) 592051 (125-27021) 592053 (125-27023)

There was also an ESCGP-1, ESX13-125-0017.

The site has an Act 9 sign at the entrance.

There was both an E&S plan and a PPC plan on-site. The PPC plan only lists Joe Steele as a DEP contact; it should include the WQS and Pittsburghï¾’s emergency telephone number (412-442-4000). Also, the PPC plan does not mention what to do in case of a spill and this is obviously should be a very large part of their planning.

The E&S controls were mostly as the E&S plan called for. We did find some minor variations and the E&S will be edited to reflect those changes.
