1860352 2010-02-02 No Violations Noted Well staked, site not constructed. Spoke with Mark Hopkins, operator representative, on 02/03/10 regarding the E&S plan. Specifically the Department requested that the plan be revised to show silt fence backed with hay bales below the site, and that the access road have road fabric laid down and clean durable rock used along the entire access road due to the proximity to the residences and the shared portion of the existing driveway. Mr. Hopkins agreed to make these revisions to the E&S plan. The E&S plan as compiled should effectively minimize accelerated erosion & sedimentation provided the plan is implemented and maintained properly.
1950268 2011-02-04 No Violations Noted At the time of inspection, rig onsite actively drilling. Observed that site was approved for waiver of distance to a stream. Site Id sign and ES plan were not directly visible due to heavy snow falls, immediately called Mark Hopkins of Kriebel Minerals whom stated sign would be re-placed in a different location today. No accelerated erosion or sedimentation issues were noted, but it was observed that bmps were improperly installed: hay bales should be installed behind filter fence, rather than in front of. Although it appears operator has adequate controls (filter fence, hay bales, and brush/grub) to prevent a possible pollution event from stream/offsite. One below grade pit onsite with adequate freeboard, although it is recommended operator install a diffuser to reduce the amount of residual dust that accumulates along/outside pit. Access road adequately rocked and maintained as outlined in ES plan, as it lies closely between several residential homes.
1950499 2011-02-02 No Violations Noted Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Pit covered with 30 mil plastic and security fence. No freeboard issue. Falcon was contracted. Stratigraphy consisted of coal, ss and sh. Conductor pipe set at 42﾿ (13 3/8﾿ size). 1st stringer (9 5/8﾿) set at 294 ft. 4 cement attempts made on stringer. First cement attempted 125 sks class A and no returns. Second cement attempted 120 sks with basket at 120 ft. Third cement attempted 100sks class A. Fourth attempt used 110 sks with gypsum base. Water located at 160 ft, 170 ft, and 220 ft. Anticipated depth to 4900 ft.
1952834 2011-02-17 No Violations Noted site Id sign and ES plan present. Well drilled, but not equipped for production. Temporary ES controls installed and functioning, although hay bales are to be installed behind filter fence. Access road stable and adequately rocked. Residential driveways and township road are being kept clean. One lined below grade pit located onsite. Site was actively being regraded and pit contents were being land farmed. IMMEDIATELY CONTACTED Mark Hopkins of Kriebel WHILE ONSITE to discuss concerns with land farming technique being undertaken at this site, as it appears operator was ﾿pushing dirt into/ on sides of pit.﾿ Mark stated he would be onsite ASAP, and confirmed that this is the method used for mixing of soil during land farming activities. Department recommended that operator ensure all the cuttings are incorporated into a min. of the top 6in of soil at 1:1 ratio and that the ground is stable enough to hold mixture (ie no recent thaw, nor frozen, or wet soils).
2038291 2012-01-30 No Violations Noted At the time of inspection, well drilled and in production. Brine tank surrounded by rock lined earthen dike. Access road and well pad adequately rocked.

Site re-graded and appears seeded/mulched with observed growth, although uniform (>70%) vegetative cover could not be determined due to snow on ground. No accelerated erosion or sedimentation issues noted, appears temporary bmp's have been removed. A follow up with be conducted in early spring to ensure adequate uniform vegetative cover is established and maintained. No violations noted.
