Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
1520858 2006-02-24 No Violations Noted Well spudded 02/22/06. PIT OK/E&S OK
1734512 2008-08-21 No Violations Noted Drilling completed on 02/27/06. Operator permitted to drill deeper and spudded on 06/12/08. E&S plan implemented properly, no accelerated E&S noted on site or road. An exposed earth diversion ditch was recomended to be rocked so as to stabilize the dicth. Operator should continue to monitor the site for accelerated E&S until final restoration is completed.
1804680 2009-06-12 No Violations Noted Drilling was completed on 06/16/08. Noted that the well pad had been reclaimed, and that the vegetative cover met the 70% uniform coerage criteria. Adjacent to the former well pad an above grade impoundment had been constructed and contact with the operator confirmed that this impoundment was for fresh water and that was the water currently in the structure. Observed numerous insects and frogs inhabiting the water contained in the impoundment. The Department recommends that the operator empty the fluids from the structure and regraded and seed & mulch the area as soon as possible to complete final restoration activities at the site.
1847637 2009-11-04 No Violations Noted Well is equipped for production. Brine Tank is in sound condition. The well tag is present. The vegetation is well established. There is no evidence of accelerated erosion or sedimentation on the access road or well site.
2160619 2013-04-08 No Violations Noted Well site was restored and in production. One brine tank was heavily corroded and missing plug from uppermost connection hole. No E&S issues were evident. Department recommends operator check integrity of brine tank and remove if no longer usable or in use. Plug should be replaced if tank is kept in operation to prevent potential leak of production fluids.
