1827387 | 2009-08-26 | Pending | Inspection conducted with oil &gas inspector Rick Hoover.
Well is equipped for production, but currently shut in. Well continues to flow artesian from 8 5/8﾿ surface casing hanger creating a pool of fluids around the well equipment and across a portion of the site. Further investigation to be done by Mr. Hoover (abandoned well, plugging required, etc.). |

1827389 | 2009-08-25 | Pending | Well tag in place. Well is equipped for production (shut in).
Access road was stabilized with vegetation (basically grown shut). Site was stabilized with vegetation. Well and associated production equipment surrounded by a small pond filled with what appeared to be AMD. Water sample was taken. Fiberglass tank was no longer plumbed into well. Tank appeared sound, although bottom 1-3 inches of tank was submerged in water. No holes could be seen in visible portion of tank. A large amount of fluid was flowing artisian from 8 5/8" surface casing hanger, contributing to the pooled water surrounding the well. This concern, along with the possibility of the well being abandoned if not produced in the preceding 12 months was discussed with oil & gas inspector Rick Hoover. Enforcement action pending on further investigation.

1830821 | 2009-09-21 | Violation(s) Noted | Water sample was taken on 8/25/09 from artesian flow of fluids from 8 5/8" surface casing hanger./ Administrative review of sample results revealed the following:/ 1) Discharge of industrial waste to the ground./ Sample results report the discharged fluids on the well site contained high levels of iron (26.0 mg/L), manganese (2.855 mg/L), and aluminum (0.977 mg/L)./ A ph of 5.3 was also reported./ These fluids were pooled around the well head and production equipment, and continued throughout a large portion of the site./ Based on the amount of fluids present, and the conditions on site, it appears as though the artesian flow has been occurring for a long period of time./ Since the initial inspection on 8/25/09, myself and oil & gas inspector Rick Hoover have consulted with operator representatives who indicated this well will be plugged./ Please submit any interim controls/plans to alleviate the above concern and a timeframe of plugging./ | 572332 | 401CSL - Discharge of pollultional material to waters of Commonwealth. | |

Well being plugged at time of inspection. Rig and operator representatives on site. Access road stable, with no signs of accelerated erosion. Limestone lined ditch was installed to drain some standing water from the site to enable rig access. Hay bales and silt fence was installed to allow solids/sediment to settle out. No violations found. |
