Site ID sign, E&S plan, permit all on-site. At time of inspection, well equipped for production. Brine tank sound. Access road stable. Perimeter BMPs correctly installed and functioning properly. Below-grade drill pit properly backfilled and pad regraded, stable. No accelerated erosion or sedimentation issues noted in relation to access road or well site.

Department recommends Operator seed and mulch site to initiate vegetative growth and monitor and maintain site to ensure that permanent stabilization and final restoration is achieved.

Site has been seeded and mulched with nearly 70% uniform vegetative growth. Perimeter BMPs correctly installed and functioning properly. No accelerated erosion or sedimentation issues noted in relation to access road or well site.

According to records, date of TD was 2/26/2013 hence Operator has until 11/26/2013 to achieve final restoration of site and submit well site restoration report.

Department recommends Operator places well tag on well head as per current regulations prior to full restoration of site and continue to monitor and maintain site to ensure that permanent stabilization and final restoration is attained.
