Observed no accelerated erosion & sedimentation with respect to the access road or well-site. The operator should evaluate all erosion controls as the snow melts for their integrity and functionality.
1865723 2010-02-23 No Violations Noted Based on record, permit was approved by 11/03/09. Well sign and E&S located at the entrance of site. According to Turm representative, well completed during the week of February 15th to the target depth. E&S controls were in place. Pit was aligned with plastic. At the time of inspection, no IW and E&S issues at the site. No signs of fluid or gas leakage around well head. Hydro frac was being scheduled for the well.
1873200 2010-03-26 No Violations Noted drilling complete and equipped for production, site rough graded, not seeded, no open pits
1900052 2010-07-21 No Violations Noted At the time of inspection well was properly tagged, drilled, and in production. Access road stable. Brine tank ok. It is recommended that additional rock be added to the well pad, as settling in low areas was observed. No accelerated erosion or sedimentation issues were observed in relation to access road or well site. Filter fence was properly installed and functioning, although it is recommended that operator continue to closely monitor structural integrity of bmp to ensure continued functioning ability. Site was regarded, seeded and mulched with near 70% successful vegetative growth exhibited. Observed small bubbles coming out of annulus and approximately 900psi of pressure on well head (matter will be referred to OG Inspector Paul Wyrwas for further investigation). Spoke with WQS Ed Ritzer while onsite, about observations whom consulted with OG Inspector Supervisor and concurred with myself to refer to OG Inspector. (Pictures and video were taken).

2113752 2012-10-24 No Violations Noted E&S controls and well tag were present. No E&S concerns were present at the time of inspection. Gas well was in production.
2157357 2013-04-08 No Violations Noted At time of inspection, well in production. Well tag present, legible. Brine tank sound. Access road in good condition. No erosion or sedimentation concerns observed. Department recommends Operator continue to monitor and maintain access road and well site as necessary.
