1391200696 | 2014-01-31 | I was on site to perform an inspection on this well in regards to complaint #4589. Complainant states that there is oil in her gas lines and she is afraid that it will get in her water supply. Dominion is on location purging the gas lines and the Health Dept. recommended she call the Division of Oil & Gas because there is a gas well within the vicinity that was drilled a year ago. I talked with Andy Smith, (330-831-1553) C & T Energy Pumper, who stated that the sales line for this well may have sent oil into the Dominion gas line. He has shut down the four wells (other wells #25710, 25770 and 25817 ) that sell into this sales line. I performed an inspection of this well and found a fully fenced in pump jack in good mechanical condition and contained all the proper identification, valves and annular gauges which showed no pressures on any casings but the production casing. The lease road was stable, plowed and the tank battery was fully fenced, contained the proper identification and had a SPPC dike. The tank battery is fully fenced and is shared with well #25828 and 25770 and contained three 100 bbl production tank, two 50 brine tanks and a poly brine tank. It also contained three vertical separators. All the equipment has bull plugs and locks. There are no violations at this time.
1704519687 | 2011-01-10 | I performed a rig inspection on Advent rig #1. The rig was drilling the surface hole and the drilling depth was 681' drilling on fluid. The ID sign was posted and a fence was around the drilling pits. This inspection satisfies 1501:9-9-03. There were no violations at this time.
-1735506950 | 2011-01-05 | Advent Drilling has started moving equipment on site and have dug the pits which are fenced. The permit information is posted at the lease entrance.
-547360048 | 2011-01-28 | On 1/27, Hotwell logged and perforated the well with 20 shots from 4343' to 4395'. On 1/28, Superior Well Services was on site to frac the well. This slick-water job started with 1000 gallons of 15% iron acid. Superior pumped a total of 88,500 gallons of slurry with 65,500 lbs of 20/40 sand. The formation broke at 1153 psi with an average treating pressure of 2004 psi at an average rate of 39.8 bpm. The initial shut in pressure was 1406 psi and the five minute shut in pressure was 1367 psi. Superior rigged down and the wellhead was set up to flow back into the open pit.
-760252295 | 2012-01-04 | The site has been reclaimed and vegetation is established. Identification signs have been posted at the wellhead and at the tank battery. Locks have been placed on the wellhead gate and the tank battery gate. The natural gas meter run at Mogadore Road has been fenced off. The Preliminary Restoration is PASSED. |
