1207941949 2008-04-10 On site with Orange fire department. There is oil around the well head, operator has mixed peat into the oil around the well head. A fine mist of oil is on the ground around the well head and on the well head fence. The affected area from the oil and brine mist is: 15' from the fence to the east, 2 feet from the fence to the west. The brine and oil flowed to the south in a cone shape behind the well head fence. Distance approximately 35' feet. The leaf litter behind the well head was stained brown. The area behind the well is a drainage swale into a stream. A seep to the east of the well head empties into the swale as well. It doesn't appear that the oil made it to the stream. The seep and water on the ground had iron floating on the surface. When I disturbed the metallic slick on the standing water near the stream it broke into squares. I called Randy Hall and left him a message. Randy Called me back that night and left me a message that he will be contacting Dan Horning. I called Dan Horning, left him a message. Dan returned my call, will be out in the morning to continue clean up. Will place boom in the small stream to the south of the well. Mix peat into the ground to soak up any oil and brine. Will also clean up peat around the well head. Leak came from the stuffing box on the well.
1210800421 2008-05-02 On site to check the status of the oil spill clean up. Operator has placed more diapers on the ground around the well. Original diapers have not been cleaned up. Peat has been placed behind the well. Additional peat has been placed around the well head. Diapers will need cleaned up. Peat will need to be cleaned up. Referred site to Norbert Lowder.

679005835 2012-01-05 On 01/05/2012, DNR Inspector Richard Skidmore completed an urban final restoration well inspection for DNR Inspector Ray Wallace. All production and storage structures, supplies, and equipment have been removed. The site has been graded and seeded to its orginial condition. Inspection passed.
-932399174 2008-04-11 On site to check the status of the spill clean up. Operator has placed a boom across the stream to the south of the well head. Operator has put down peat to the south of the well head to absorb any spilled brine and oil. Tested water that was standing in puddles to the east and west of the well head. Water in the puddles when tested for CL- on low range Quantab had Cl- great than 600ppm. Quantab read 9.4, which is above the scale on the bottle. Tested water at the head of the stream, no chlorides in water. Operator had 5 diapers remaining in a bag by the well head. I placed the diapers on the small puddles of brine around the well head. Called operator, Dan Horning is going to replace the diapers Monday.
