Checked casing program for what casing was set 72' of 11.75 conductor cemented on 7-23-10 no problem cementing,85/8 casing cemented on 7-23-10 no problems with cementing.

Phone to Mrs. Gallou informed her that the well meets spacing and no problems encounter while cementing of the conductor and surface casing.

1322771487 2011-12-01 Received complaint from resident 7 doors away of petroleum odor in water when opening spigot on outside of house. Scanned area with gas meter and obtained following readings. O2- 20.6/ LEL - 0/ H2S-0/CH4IR/ CO - 0. Company checked well pressure prior to 12:00 Noon and recorder 1 Lb pressure.
1323186307 2011-12-02 Received notification from Norburt Lowder on 12/01/2011 of complaint of petroleum smell coming from outside water spigot when it is turned on. Residence is located at 31107 Cedar Road, Mayfield Heights, Ohio. Residence is located West of the oil and gas well, #34-135-2-2161-00-00. Well and tank battery were inspected, slight gas odor in air detected by smell when entering inside well head area. Used gas meter at waist level and around connections and fittings. The readings from the meter were as follows: O2-0/LEL-0/H2S-0/CH4IR/CO-0. Well head area and tank battery were inspected by the inspectors on site. Well is in compliance with 1501:9-9-05

-2013994434 2014-08-18 I inspected this producing unit as part of routine inspection activity. The well was completed in 07/28/2010. The unit is producing and production reports are current. The producing unit is accessed via a dedicated, gravel-paved access road which extends north from the terminus of the current residential driveway. The access road is level, and well drained with no signs of erosional deficits. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system and aboveground storage tank (AST) battery in separate board-on-board enclosures. The valve fittings, connections, and other production appurtenances appear to be well maintained and are free from signs of leakage or other problems. The battery is comprised of 1 steel 100-barrel production tank, 1 25-barrel poly brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. There is secondary containment at the battery and the interior areas are dry. I did not observe any signs or evidence of leakage, spills, or problems. Unit identification placards are present at both the battery and wellhead and are in good condition. I observed no issues or problems requiring attention at the time of my inspection.
