1398940324 2013-01-14 The producing unit is comprised of the following elements: a tank battery with two 100 barrel (BBL) tanks, an in-ground brine tank, a mechanical separator, and a wellhead valve assembly. A mechanical plunger lift is affixed to the wellhead valve assembly. The tank battery and wellhead are enclosed in separate, 6-foot board-on-board enclosures. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. The tank battery secondary containment is adequate. I did not observe any leaks, spills, or other signs of contamination. The gates and loadouts are locked.

634276681 2015-02-24 I inspected this well as part of routine inspection activity . The wellhead and battery are located an adjacent yard, south of the Masonic building, next to the parking lot. Connections and fittings at the wellhead and battery are secure. Enclosure gates are locked. Signage is present and accurate. Load-outs are plugged and secure. Production reports are current. At the time of this inspection there is 12 to 15 inches of snow cover, which obscures some of the surficial features. The areas surrounding the wellhead and battery are comprised of a grassy tree lawn adjacent to the asphalt parking for the Masonic Hall. I did not observe any signs of impacts relating to the well on the surrounding areas.
