1355147951 2012-12-10 I conducted a production well inspection as a compliant from the landowner Mary King. My inspection found the well equipped with a plunger lift unit with a disconnected flow line and a bull plug installed at the tubing. The tank battery had no identification sign and small trees. It contained one, rusty, 50 barrel production storage tank. There was a disconnected 2 flow line lying next to the tank battery. I took photos of the well head and tank battery. I researched our database and found no production reported since 1994 by Belden and Blake Corporation. I called Petrox, Inc. at 330-653-5526. I was told that Petrox, Inc. sold the well to the landowner. I obtained a copy of the paperwork that showed the transfer to the landowner Helmi Peltola on 12/6/1994. I sent the information to Columbus to update the well from producing to domestic.
1357652418 2013-01-08 I received a confirmation letter from Columbus correcting the well status from a production well to a domestic well. My research found a bill of sale, an acknowledgement letter from ODNR Division of Oil and Gas, and a Request for Change of Owner form showing the well had been sold to Mr. Helmi Peltola in 1994, but two of forms listed the wrong permit number as 34-155-2-0678-00-00. I informed Mrs. King of the correction and emailed the letter to her.
325870960 2015-06-03 I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a pumpjack. A one inch domestic gas line comes off the well's 4.5 casing. The tank battery consists of a 100 barrel production tank and a 50 barrel brine tank. A dike surrounds the tank battery. There is no identification sign posted. The lease road is in stable condition. I called and spoke with Jim Adams of J Well. I instructed Mr. Adams to post the proper identification sign.
-668805084 2015-06-03 I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a pumpjack. The tank battery consists of a 100 barrel production tank and a 50 barrel brine tank. There is a dike around the tank battery. I observed no identification posted. The lease road is in stable condition. I researched our RBDMS database and found J-Well took ownership on 2/23/2015. I called Jim Adams of J Well and instructed him to post the proper identification.
