1423744474 2015-02-11 Performed pre-transfer domestic well inspection on 02/11/2015 at the request of Kathryn Smith. The well is equipped with a swedge, T, and pressure gauge. The pump jack and all production equipment has been removed with the exception of one small drip tank. No well identification is posted. The landowners, Mr. and Mrs. Matus arrived during the inspection and confirmed plans to utilize the well. A line has been roughed-in to the house and outbuildings on the property and will be tied-in as weather allows. No problems were noted at the time of the inspection. Well can be transferred to landowners.
249005161 2015-04-29 Inspected well on 04/29/2015 at the request of Kathryn Smith for an exempt well pre-transfer inspection. All tanks have been removed. The well is equipped with a wellhead, swedge, T, gauges, regulators, and a drip. The well was flowing at the time of the inspection. Mr. Matus stated that a licensed plumber had tied the well in for domestic use. As the well is now equipped for domestic use, the well can be transferred to the landowner. No problems were noted at the time of the inspection.
431226499 2013-10-10 FOLLOW UP TO NOV # 1379105813 Performed follow-up inspection on 10/10/2013. Tank battery consists of 1-210 bbl tank and a production unit on a gravel pad within an earthen dike. Wellhead is equipped with a pump jack and an electric motor. No well identification is posted on either the tank battery or the wellhead. No insurance information has been provided to the state.
