Well was not properly identified, no production records have been filled with DMRM since 2008, production tank had leaky valve which had allowed brine to escape. Brine scalded area was 4x15 feet and measured 6797+ ppm chlorides with a quantab. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Valve on production tank shall be replaced or repaired. Contaminated soils shall be removed, reseeded and mulched. Well shall be produced, plugged, or placed in temporary inactive status.
-1314503054 | 2014-04-10 | Compliance Notice Follow-up -276953186: There has been no resolution to the issues. There is no identification posted at location and production still has not been turned into the state since 2008.
1318454060 | 2013-01-31 | Follow up to 1312821519. The old 100 Barrel tank close to the road is still on site. The tank is still leaking a small amount of fluids from the valve at the bottom. The valves on the tubing are open. There is no ID at the well or tank.
1373124298 | 2014-04-21 | Follow up to -276953186. 100 Barrel steel tank still by road, does not appear to be leaking but contamination not removed yet.
-1383881405 | 2013-04-08 | Follow up to -276953186. The tank has been emptied and is no longer leaking brine. The contaminated area around and below the tank still has not been remediated.
1397241900 | 2014-04-11 | I spoke with Mike Bendimen and he stated that he plans on tying this well and the adjacent well together and using one tank battery. He will have the plastic tank and contamination cleaned up within thee next week. Once he finishes the cleanup he will begin working on getting the well back into production.
1457355586 | 2012-04-25 | I spoke with Mike Beridum (304.677.3750) of Blackrock Energy about bringing the well into compliance. Extension was granted for 6/1/12.
-1480758342 | 2012-06-08 | Began project to rebuild dike. Spoke with Mike Benidum of Blackrock energy about spacing distances and dike dimensions.
-1503612171 | 2015-03-11 | Follow up to -276953186. Scald area remains below tank alongside road. I called Mr. Benedum of Angus Energy and left a message about resolving the issue.
1579770915 | 2012-02-13 | Compliance notice follow up. There is still no identification posted at the well. The production tank is disconnected.
1648293923 | 2014-01-28 | Follow up to -276953186. 100 Barrel steel tank situated near road does not appear to be leaking. Still no production recorded.
1655362908 | 2013-08-19 | Follow up to 1312821519. No work done at dike, still contamination around tank. The identification on the tank has fallen off. The valves on the production casing is closed and the valve on the tubing is open. No production has been reported to the division.
-1937169225 | 2012-07-23 | The old 100 Barrel tank close to the road is still on site. The tank is still leaking a small amount of fluids from the valve at the bottom. I tested the water dripping out of it and it contained 6787+ ppm Chlorides. I spoke with Mike Benedum of Blackrock Energy about bringing the well fully into compliance.
-207223029 | 2014-04-30 | Follow up to -276953186 and 1312821519. The well is overgrown with vegetation. The valves on the production is closed but the valve on the tubing is open. Work has been done around the old leaking tank, but it still remains on site and there is still contaminated soils. I spoke with Mike Benidum of Black Rock and he states a new meter will be put on the well sometime next week and the tank should be removed after that.
246545987 | 2013-12-24 | Follow up to -276953186. The tank does not appear to be leaking brine, but the contaninated soils remain on site.
-276953186 | 2013-02-19 | The plug at the end of the load line on the bottom of the 100 barrel (Bbl) steel tank was leaking a small amount of fluids and contaminating an area below the tank roughly 15' x 4'. The fluid dripping from the valve was field tested for the presence of salt with a Quan-tab and found to have 6508+ ppm Chlorides. There was no dike around the tank and the tank was situated roughly 20 feet from the county road and in a way so that a potential spill may affect the traveled portion of the road. All contaminated soils shall be removed and disposed of properly. The affected area shall be seeded and mulched and steps shall be taken to encourage vegetative cover. Plug in load line shall be repaired or replaced. Tank(s) shall be placed inside a dike with a capacity enough to hold 110% of the largest vessel and meeting all current spacing requirements.
