1331052107 2012-03-01 arrived on location and found well idle , no id posted and buried saltwater tank full. Within 30 days post id , empty saltwater tank, and place well into production or apply for plug permit.
919270074 2014-05-02 On the day of my inspection following up on NOV-1331052107 and complaint id #3631, I witnessed that the well was equipped with 4.5 inch casing, 1.5 inch tubing, a plunger lift system and a domestic gas hookup. Production equipment included a 210bbl tank, a concrete brine vault, a vertical separator and a 2 inch meter run with a .5 inch orifice and a chart recorder. Identification was not present and production records are current. Made phone call to Jeremy Caldwell of PAC Drilling, operator of well and I stated that identification needs to be updated to clear the current violation on the well. Jeremy stated it would be up by the end of next week.
936631306 2014-04-09 On the day of my inspection following up on NOV-1331052107, I witnessed that the well was equipped with 4.5 inch casing, 1.5 inch tubing, a plunger lift system and a domestic gas hookup. Production equipment included a 210bbl tank, a concrete brine vault, a vertical separator and a 2 inch meter run with a .5 inch orifice and a chart recorder. Contamination has been cleaned up and with production records current and meet the minimum for 2013. No identification was present.
998489007 2015-04-22 On the date of inspection I am following up on NOV-1331052107 and complaint #3631. The well is equipped with a gas plunger lift system, 1.5 inch tubing, 4.5 inch casing, and 8.625 inch casing. The tubing valve and one casing valve are in open position. Casing valve is connected to a domestic line. Production equipment includes a 210bbl tank, a concrete brine vault, and a vertical separator. Identification is present on the tank and the well head. Production records are current through 2014 and have met the minimum requirements for 2013 and 2014. The well is now in compliance.
