1329490908 2012-02-17 On 02/17/2012, DNR Inspector Richard Skidmore conducted an production well inspection. My inspection found the identification sign was improperly marked with another permit number, County and Township location, and well owner name. There was a small gas leak coming from the plunger unit (rabbit). I immediately contacted Joe Glista (Energy Resources America) at his cell number (330-720-8137) to inform him of the issues with this well site. Joe said he would send out his well tender to repair the problems. A follow-up inspection will be conducted by 02/21/2012 to ensure compliance with state regulations.
1675487347 2008-01-02 I performed a random field inspection. GPS'd both the tank battery and the well for the first time. The ID sign was posted. The tank battery was in good shape. There were no violations at this time.
-1776120983 2012-02-21 On 2/21/2012, DNR Inspector Richard Skidmore completed a production well followup inspection. The identification sign was corrected with the proper information, a bull plug was installed into a ball valve at the base of the storage tank, and the leak at the plunger lift was repaired, as specified from the initial status check on 2/17/2012. The lease road, well head, and tank battery locations are in compliance with 1501:9-9-05. Inspection passed.
