1326818896 2012-01-12 On 01/12/2012, DNR Inspector Richard Skidmore completed an urban preliminary restoration well inspection for DNR Inspector Ray Wallace. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
1349526520 2015-02-27 I inspected this well as part of routine inspection activity. The wellhead and battery are located in the front of a single-family residence at the referenced address. Access to the well and battery is via the shared residential driveway. The wellhead plunger lift and battery are in separate board-on-board enclosures which are locked. The wellhead is comprised of 1 100-barrel steel lowboy production tank and 1 100-barrel steel lowboy brine tank, separator and Dominion metering run. Fittings and connections are secure and free from signs of leakage or problems. The load-outs are secure, plugged, and locked. Process safety gear is present. Signage is in place and current. At the time of this inspection there are 15 to 20 inches of snow cover in the area which obscures some surficial elements from observation.
1415370095 2014-11-06 I inspected this unit pursuant to a complaint of odors (rotten egg, sulfur) from residents of the neighboring property (1120 W. Hill Drive). Upon my inspection I found the wellhead plunger lift to be stable. The valves and connections are free from signs of leakage. I did not detect any odors or register detectable levels of explosive gases (LEL), carbon monoxide (CO), or hydrogen sulfide (H2S) at the wellhead. I did not observe any problems or signs of spill or leakage at the tank battery. Valves and connections were secure. The entry gate was locked. I did not detect any odors or detectable levels of gases (LEL, CO, H2S) within the battery or surrounding area. I spoke with the residents at 1119 W. Hill Drive and they did not report any odors or problems. I also spoke with the residents at 1120 W. Hill drive (original complainant). They reported smelling rotten egg type smell earlier in the day, but nothing currently. I did not detect any odors or record any gas detections at this residence. I advised the complainant that I would return and reevaluate in the event of a recurrence of the odor.
