| 1427974559 | 2015-04-01 | Mr. Steve Sapio of Wayne National Forest, Inspector Matt Fulk, and I performed an inspection on this well in response to a request from the Wayne National Forest Field Office. Upon inspection, it was observed that the well is capable of production and is producing. The well consists of a pumping unit standing over 13 conductor, 10 surface casing, 4.5 intermediate casing, and 2 tubing. The tank battery for the well consists of a horizontal 100 bbl production tank with a small plastic brine knockout tank. There is a brine disposal line leading from the knockout tank to a disposal pit slightly downhill and to the east of the tank location. The disposal pit measures 45' x 20' and there is an overflow point where fluid has breached the pit in the past. The disposal pit is void of vegetation and a field sample from the disposal line yielded a chloride reading of 6545 ppm. Disposal of brine in this manner is illegal and must cease immediately. The operator shall route the disposal line into the 100 bbl production tank or install an auxiliary brine storage tank from which brine can be hauled away and disposed of properly. The disposal pit must be closed, put back to original topography, and the soil must be remediated to the point where woodland vegetation can grow again. The operator will have 30 days upon receipt of this notice to bring this location within compliance. | ||||
| -1575322853 | 2015-05-06 | Upon inspection, it was observed that the brine disposal line has been removed from the disposal pit and all produced fluids are now being stored in the production tank. Mr. Schultheis was on location during my inspection and he stated that he has a bulldozer lined up to come in and close the pit next week. I will continue to monitor the progress of the cleanup. | ||||
| 1761448609 | 2015-06-04 | Upon inspection, it was observed that the brine disposal pit has been closed and the area has been put back to grade. Mr. Rob Schultheis was present during the inspection and stated that lime and fertilizer was mixed in with the soil to counteract any residual chlorides. The location has been seeded/mulched and vegetation is growing in throughout the entirety of the former kill area. Photos were taken to support the findings of this inspection. The compliance notice dated 4-2-2015 can be closed out. |
