1292526862 2010-12-16 I received a call from Mr. Fox(330-398-9065) about getting a water well sample. I stated that he was mad about all the phone messages telling him when the water sample crew would be out to take his water well sample. I told him to call me when he and his wife have a time when the water sample is to be taken. I first said that anytime next week or the week after Christmas. He would call me either tomorrow of Monday, 12/20/10 about the time. I called John Howell Tool Pusher on(330-301-0769) Wildcat rig #1. He said they would be moving to this location at the earliest by next Tuesday. The rig would shut down for Christmas on 12/22/10. I felt that I was wrong on what I told Mr. Fox. I called Mr. Fox back and told him that the water well samples needed to be taken either Monday or Tuesday, 12/20/10 or 12/21/10. He thanked me for calling. Steve Opriza had a phone conversation with Mr. Fox and told him to call Everflow to get a date and time to have his water well sampled. Mr. Fox stated on the phone to me that he had not called Everflow. The letters and phone messages from Everflow were not nice as told to me by Mr. Fox. Everflow had called numerous times to set up a date and time for taking the water sample. No response from Mr. Fox. If I have not received a phone call from Mr. Fox by next Tuesday, 12/21/10, then I will contact him.
1292604002 2010-12-17 I called Mr. Fox(330-398-9065) to talk about the water well. I told Mr. Fox that ODNR would take his water sample. He agreed to have ODNR take the sample. He stated that he or his wife would be there around 4:00PM, 12/20/10. I called Kyle Tharp to meet me in Boardman next Monday. We both will go to the Foxes house.

1297360562 2011-02-09 I partial witnessed the solicitation of the drilling pits. There will be a total of 4 loads of material dumped on the 2 pits. There were no violations at this time.
1302271457 2011-04-08 The top soil is still piled west of the wellhead. The process of building the tank battery area has started. The ID sign is posted at the wellhead. The area is very muddy. No production from the well.
1306954815 2011-06-01 I received a call from Kyle Tharp about a water well complaint at 96 RockeWay in Boardman. He wanted to know if Everflow had taken a water sample before the well was drilled. I called Les Dundics of Everflow. They had taken a water sample even though the well was farther than 300 feet. Les emailed me the water well report and I forwarded it on to Kyle. Kyle tested the well and preliminary reports shows no sign of a oil water well problem. The water well had bacteria in it. The final test results will be in 7 -10 days.
1308133211 2011-06-01 I received a call from Kyle Tharp about a water well complaint on 96 Roche Way Street in Boardman Township. Ms. Carolyn Wilk thinks her water well is contaminated by this well. Kyle will let me know when the water samples analysis is done. I inspected the well in question and found that the well has an ID sign and is not yet in production. The tank battery area is not built yet. There is an orange fence around the wellhead. There were no violations at this time. This complaint continues.
1308759503 2011-06-22 The tank battery area has been built. The large pile of top soil is still on the drilling location. Dan's Excavating is on the location performing restoration work. There were no violations at this time.
1309183649 2011-06-27 Received phone call on 06/27/2011 at 10:02, from Mary Alice Hawkins of 121 RocheWay, Boardman, OH 44512 in regards to restoration of well site and fencing of well head. Called George Strawn (330-727-1614) of Everflow on 06/27/2011 at 11:12 to ask about status of restoration. George Strawn stated that they were removing temporary access road today, they just got flow line to tank battery laid down, and they intend to get top soil graded by Friday 07/01/2011. I conducted field inspection of well head and tank battery on 06/27/2011 at 15:30. Site is very muddy. Mound of topsoil remains to East of well head. ID sign in place on well head. No fence around well head. Tank battery in place. Dike constructed with gravel bottom. One 70 bbl stock tank, one poly brine tank, one separator. Flow line laid out. No fence or ID sign on tank battery. Well is not in production. Weather conditions delaying progress of restoration. I called Ms. Hawkins (330-853-5461) on 06/27/2011 at 16:05 to report the findings of my inspection. Complaint resolved. No violations at this time.
1313423655 2011-08-10 Gate with ID sign installed at Margaret Street. Board on board fencing around well head with ID sign posted. Top soil graded out, area seeded, straw down, good grass growth around well head. Chain link fence installed around tank battery with ID sign posted. Containment dike and surrounding area, seeded, straw put down, no grass growth. Does not pass preliminary restoration at this time.

-2036138988 2010-12-15 I received a call from Kathy Lynn(Everflow Eastern) about the Fox's(Lawrence and Shalyne Fox, 103 RocheWay, Boardman OH 44512) water well. She is going to mail out a certified letter today to the Fox's to let them know that Everflow will be out on Monday 12/20/12 between the hours of 8:00AM to 6:00PM to collect the sample from there water well. The Fox's are to call Everflow on the time they would want the water well sample taken. Inspector Carl Roberts will accompany Kath Lynn to collect the water sample.

-773166237 2011-06-15 Kyle Tharp emailed me that the water well tests showed no oil related to Ms. Carolyn Wilk well. He will write a letter to her stating the results. Kyle stated bacteria in the well is very bad. This complaint is resolved.
