1285072154 2010-05-17 Orphan well inspection. I took Scott Means USFS to well site. Well is leaking small amount gas from slips. Well tubing has a bullet hole below the valve. Took pictures.
1331169529 2012-03-07 Witnessed bottom hole plug. Bottom plug set at 1525 with 47 sacks through tubing . Production casing shot at 958 feet.
-1334490529 2012-03-06 5.5 inch casing set at 1484'. Drilled to 50 feet below 5.5 inch casing at 1535 feet still drilling cement. Drilling with 4.75 inch bit. Circulate hole with fresh water.
-1391594572 2010-08-30 Orphan well referral follow up inspection. USFS has installed orange safety fence around wellhead with 3 metal posts set in a triangle. Well has been marked with red marking tape. Slight gas leak was observed at base of wellhead. No gas was observed coming out of the bullet hole in the 2 tubing. Took pictures of the well site and the access road. 15-20 trees will need to be cut down to allow access to the well along the current ORV trail. Tree size ranges from 2 to 12 in diameter.

-1542938037 2012-05-10 Final restoration inspection with Scott Means USFS. Good vegetative cover. No erosion. Well site and access road are part of the USFS Hanging Rock ORV Trail system. A waiver form was requested due to the roadway being left and not reclaimed.
-1555048930 2009-03-03 Received request for inspection on 2-25-09 regarding the status of this well from Rich Jones, Lands Program Manager USFS Ironton Office. Wellhead is located at an intersection of ORV trails at the Hanging Rock Trailhead. Well is idle. Well is not connected to production equipment or lines. Wellhead consists of 8 5/8 inch surface casing, 5 1/2 inch production casing with a swedge to 2 inch with a 2 inch valve attached. 2 inch valve is closed. Wellhead has been damaged and the top of the swedge is bent. The 2 inch coupling on top of the swedge is starting to break due to the damage. No gas leaks were detected at the time of inspection. A review of the scanned images for this well revealed that the well has been plugged back on 1-10-1979 to the top of the Big Lime. Pictures were taken.
1647275188 2012-03-07 Plug through tubing ran to 1525 feet. Circulated water prior to pumping cement. Pumped 5 sacks then had problems with water jet. Had to circulate cement back out of well. Repaired water jet. Achieved good circulation and pumped plug 45 sacks. Pulled 1.5 inch tubing. Appalachian Well Survey ran a wire line bond log looking for free point on 5.5 inch casing. Casing free at 990 feet. Used casing cutter at 958 feet. Casing is free.
1843426989 2012-04-30 Well site and access road are part of the Wayne National Forest Off Road Vehicle trails. Site has good vegetative cover except on trail (which is in use). Contacted Rich Jones Re: Landowner Waiver for the road being left in place.
-2030054491 2012-03-09 Big Injun plug 874 feet to 724 feet. Load hole with gel prior to pumping cement. Ran tubing to 874 feet. Achieved circulation prior to pumping cement plug 43 sacks. Pulled 7 joints tubing for bottom of surface casing plug 641 feet to 441' 62 sacks cement. Good circulation during plug. Pull 16 joints tubing for 100 foot to surface plug, 33 sacks cement. Circulated prior to pumping cement. Good cement at surface.
203274505 2012-03-06 Barry Bowers called and said they are at 1485 feet. He said that it drilled solid for 50 feet then washed fine clay then was free. He hit solid again at 1480 feet.
