Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1612122817 2015-03-31 On the date of inspection I was on site to inspect the well that was currently in Production Well status in RBDMS. The well was added to the system in fall of 2014 due to landowner complaint #3490. I found 11 inch conductor pipe sticking out of the ground 2 feet above surface. The conductor was filled with cement; however, there is a pin sized hole in the cement where I was able to hear and detect the odor of gas. I held my personal protective Honeywell PhD6 gas monitor over the hole and the alarm went off measuring CH4 at a level of 1.3 (Danger at 1.00) and the LEL at a level of 79 (Danger at 20). There was also a valve that was allegedly installed by the landowner but is now corroded. The well is located in the Short Creek floodplain. I will refer the well to the Orphan Well Program for further evaluation.
