1285180766 2010-09-22 Gavan Byrd cc: me on a email to Kelly Barrett in the Columbus office. Mr. Byrd stated that the adjustment with the API numbers have been corrected. He is working with Kelly about getting the past production reports corrected too. There were no violations at this time.
-2133141141 2012-03-07 On 3/7/2012, DNR Inspector Richard Skidmore completed a production well inspection status check for DNR Inspector Chris Grimm. This well shares a lease road with permit number 2-2266. The lease road, well head, and tank battery are in compliance with 1501:9-9-05. Inspection passed.
544887883 2010-09-15 I inspected this well per request from Jay Cheslock. There was a problem with the API numbers for this well. One number was 0088 and the other was 335. I called Steve Opritza(614-265-6442) and told him one well was Columbia Gas storage well and the other(0088) was a Berea well. Steve made the changes to this well.
-568846401 2010-09-20 I called Gavan Byrd in Oklahoma City(405-935-1353) about getting production to reflect this API number and not the 0088 API number. He stated that he will get things changed. He will email me with the change takes place.
936765631 2010-07-06 I performed a random field inspection. I GPS'd both the tank battery and the wellhead for the first time. I continued to walk around both the wellhead and the tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time.
