1226951599 2008-10-22 A follow-up inspection revealed non-compliance.
-1278405710 2008-07-02 The Division of Mineral Resources Management received information that Mr. Larry Horn has been removing equipment from Bay State Exploration wells. An inspection revealed that the stuffing box rubbers need replaced. The well is leaking gas. The landowner is receiving house gas from this well. A 100bbl tank has been removed. A pump jack was not observed during the previous inspection. This well had been routed to the same tank battery as the C&G Gray #2. A Notice of Violation will be sent to Mr. Horn.
-979209251 2007-08-09 An inspection conducted on 8/9/2007 revealed that a c-clamp had been placed around the well head to stop a gas leak. At the time of the inspection, gas was found to be leaking from the well head. The well does not contain a pump jack or identification. This well utilized the same tank battery as the C.Gray #2 located alongside the road (not connected). There are no production records for this well. A representative from Kilbarger Investments state that the landowner is receiving house gas from this well. This well may have been placed in the I/O program.
