-1291669524 2008-07-25 The two areas under the oil load out lines have been cleaned up. Additional clean up is required at the tank battery. I flagged the areas and called Tim McConaha.
1405344213 2014-07-21 I was contacted by Louisville Fire Dept. Chief to visit this well and ensure proper cleanup and safety standards were applied to this well following a call regarding the smell of gas and a small oil leak. The stuffing box for this well failed and sprayed less then 5 gallons of oil inside the fencing for this well. I checked the location and the spill has been completely cleaned up from location. The stained soil has been scraped up and new gravel has been placed on location. The fence around the pumpjack has been fixed and the wellhead is now fenced in. The cleanup for the spill has been approved and there are no violations at this time.
-1543760320 2008-07-10 I received a call from Bob Bennett (330-875-9940) of the Stonebridge Allotment regarding the requested clean up at the wellhead and at the tank battery. The wellhead area has been cleaned up. The tank battery has not been cleaned up. I called Tim McConaha and repeated my recommendations from the 6-3-2008 inspection.

1612791351 2008-08-12 The tank battery fencing has not been repaired and additional clean up is still needed at the brine load out line. I called Tim McConaha.
-395979457 2008-06-03 Tim McConaha (Enervest) requested that I inspect the tank battery for violations in regard to a complaint he received from a neighborhood resident. This is an older well and tank battery, but it meets nearly all of the HB-278 requirements. I told Tim to remove the stained soil from under the load out lines, add locks to the load out valves, attach lightening arrestors to the vent stacks, and repair the strands of barbed wire along the top of the tank battery fence.
