Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1035856660 2012-10-10 On the day of my inspection I found that Wheeler Drilling set a brush/stone bridge at 1000'. They pulled 470' of 8 casing and then shoveled 4 tons of clay into the well. A pre-cast concrete plug was set at 470' and then 4 tons of clay was shoveled into the well. Wheeler Drilling shut down for the day to let the clay settle overnight.
-1280744810 2013-06-24 I received a fax copy of a signed landowner waiver for the final restoration of this well site. The pits are filled, the site is rough graded and the production equipment has been removed. All other restoration requirements have been waived by the landowner. This site passes final restoration.
1350311487 2012-10-09 On the day of my inspection Wheeler Drilling ripped the 4.5 casing at 3210', pulled 8 joints and shoveled 4 tons of clay into the well. They pulled the rest of the 4.5 casing from the well, set a pre-cast concrete plug at 2488' and shoveled 5.5 tons of clay into the well.
1484988577 2013-05-20 On the day of my inspection I found that the area had been rough graded after the well was plugged and nothing else had been done to restore the area. The landowner approached me and claimed that he wanted to build on the site and he wanted the access road left in place. I informed him of his right to waive the restoration. I contacted Mason Producing, who claims they will get a landowner waiver form out to him as soon as possible.
-1738518395 2011-04-08 Rain most of the day. Pumping well equipped for production.
-594130299 2012-10-11 On the day of my inspection I found that the fluid level was down to 200'. Wheeler Drilling shoveled 7 tons of clay to fill the well to surface. They cut the casing off 30 below grade and capped the well with a steel plate inscribed with ODNR - 3380.
