1158173973 2006-09-13 Received phone call from Dave Barndt of Clearwater Drilling about 2 abandoned wells on property at 6401 Seaman Road, Oregon, Ohio. The landowner, Noel Graham, wants the wells plugged because of housing development proposed for site. Clearwater Drilling will apply for permits to plug the wells. The landowner will pay for the cost of plugging the wells. The wells were supposedly submitted to the I&O program 5-10 years ago but he has not been contacted about having the wells plugged. 1159197134 2006-09-25 On site for completion of well plugging. Spoke to Dave Barndt about NOV's issued by Tom Benko on 9/22/06. The 4 inch casing was shot at 303 feet on Thursday 9/21/06. The 4 inch casing was removed. The well was apparently stimulated by the shot and began to produce oil later that day. Oil was contained in the plastic lined pit and an above ground storage tank was also available for additional storage. A vac truck was also notified if needed. One of the operators remained on site for the night to monitor oil discharging from the well. Dave estimated that 160 gallons of oil were produced from the well before being plugged on Friday 9/22/06. Dave was concerned that they were taking precautions needed for the well but received NOV's. Will check into situation. The operator ordered 3 yards of 9 sack grout to complete the well plug. The well was baled out and contained both water and oil. Well depth to top of cement was 164 feet. The 9 sack grout was gravity fed into the 6 inch casing and was also used to fill between the 6 inch casing and the 8 inch conductor pipe. Cement rose to the top of the 6 inch casing as well as the annular space. The well casing will be cut off below graded after a retention basin is excavated on the well site for a proposed housing development. Tom Benko will complete the plugging report for the permit
1159979884 2006-09-22 Plug well. Shot 4 inch casing at 300 feet. Fluids were at 400 feet. Flowed oil from noon till 7:00 pm. Approximately 7 bbl of oil. Total depth of hole is 1325. Ran 1100 feet tubing and pumped 125 sacks of cement. Pull up tubing and pump second load of 125 sacks of Class A cement. Displace about 1 bbl of oil during cementing.

1528900770 2006-10-03 The well site is now being developed for housing project and is fenced off from previous access. All tanks and equipment removed. The well casing will be cut off below ground when a retention basin is excavated at the well site. An ID plate will be attached at that time.
-1815674462 2006-09-21 Plugging contractor shot well at 300 feet. Fluids in well borehole at 400 feet. Excited well and flowed 7 bbl oil to surface. Bottom plug was not set priort to ripping/ sooting well. Rush permit purchased and issued.
-634253116 2006-09-20 Received call from Dave Barndt that well may be cemented in late afternoon. When arrived on site the operator was pulling tubing from well. Rods were removed earlier and were on site. Estimated depth of well was 1350. There was salt water in the hole at approximately 450 feet. A plastic lined pit was next to well for fluids. The 4 inch casing in the hole will be shot on Thursday so the casing can be removed. No plugging on this day.
