1230278358 2014-02-14 On the day of my inspection, I reviewed the Division of Oil and Gas production records and found them to be current for the years of 2011 and 2012; the well is equipped with a pumping unit above an eight and five-eighths inch surface casing with four inch production casing and two inch tubing. Upon inspection, the pumping unit was not operating. The well is equipped with a domestic gas service line. The pressure gauge on the four inch production casing read fifty pounds of pressure. The valve on the four inch production casing as well as the two inch tubing at the well was in the open position. The two inch line leads into a drip / separator / chart recorder and (1) one-hundred barrel and (1) two-hundred and ten barrel production tank sitting inside a containment dike. The valve was in the open position at the drip on the production line, and the valve coming out of the separator and into the production tanks were in the open position. The date listed on the chart recorder was February 12, 2014. There was no I.D. posted at the well or at the tank.

The requirements to meet the Division of Oil and Gas standards are as follows: 1.) Properly display identification information at the well and/or tank
