1042469038 2013-11-14 This inspection was conducted as a preliminary restoration status check. The well was equipped with a pumping unit and motor. The polish rod showed signs of recent production. The tank battery consists of (3) 100 barrel tanks and separator inside an earthen dike. An incomplete identification sticker was attached to the tank. The pit used during drilling has not been closed properly and is causing substantial erosion. The inspection revealed non-compliance with ORC 1509.072. The company will be notified of inspection results and will need to correct violations. Field Visit or Inspection
1292431809 2010-09-23 New well. Has pump jack and is pumping. New tanks set, 3 100 Bbl tanks and vertical separator set with earthen dike. Pit being squeezed in.
1901230867 2013-11-22 I inspected this location accompanied by Jeff Fry to discuss a remediation plan for the pit. The company will be notified and will need to correct erosion issue being caused by the improper closure of the pit.
