1587806121 2014-08-08 Upon inspection, it was observed that no changes have been made to this well location. I spoke with Mr. Paul Valentine in person and he stated that he is in the process of making new ID tags for all the wells on this lease. Mr. Valentine also stated that updated production records will be submitted for all of the wells on this lease. Mr. Valentine agreed to take prompt action on bringing this well into compliance.
1658318281 2015-03-17 Upon inspection, it was observed that this well is capable of production and is producing. A new meter has been set for this well, P#62145, and 66174. Mr. Paul Valentine stated that all of the wells on this lease will be metered through the same location when he is finished servicing the rest of the wells. Mr. Valentine stated that production records have been submitted to the division for all of the wells on this lease. Since the records still are not showing up on RBDMS, I instructed Mr. Valentine to mail the production records to the ODNR DOGRM Zanesville Office so they can be scanned into the system. The compliance notice for this well can be closed out.
1733778383 2015-01-20 Upon inspection, it was observed that this well is capable of production and is producing. I spoke with Mr. Paul Valentine in person and he stated that production records had been submitted for this well a few months ago, however, the records are not showing up in RBDMS. I instructed Mr. Valentine to send a copy of the records to me at the Zanesville Office and I would scan them and send them to Columbus.
-606720864 2014-07-22 Performed an inspection on this well in response to a landowner complaint. Upon inspection , it was observed that this well is capable of production and is producing. The well consists of a pumping unit standing over 6.625 casing with a gas escape packer with 2 tubing, rods, and 1 gas escape tubing. There are lines connected to both the 2 and 1 connections and valves are in the open position. No pollution or contamination issues were observed. The identification information posted at the 100 bbl production tank is faded and inaccurate. According to RBDMS production records, this well produced 6 bbl of oil and 25 mcf of gas in 2007. No production records have been submitted from 2008- 2013. P&D Valentine Oil Co. will have 14 days upon receipt of this notice to submit current production values and post I.D. at the well location.
-92135784 2014-09-03 Upon inspection, it was observed that this well is equipped for production and is producing. New identification information has been posted at the well head. I spoke with Mr. Paul Valentine in person, and he stated that he is in the process of submitting current production records for all wells on the property.
