1904828552 2012-04-12 Well was not equipped with an identification tag. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Well was equipped with a pumping unit. The valves on the production string were cracked open, the valves on the tubing were closed. There has been no production recorded since 1997. Well shall be plugged, or placed in temporary inactive status.

-491587293 2010-01-28 Inspection this day with supervisor to check status of well. Found well with pumpjack, no motor idle. Lease road very overgrown and unpassable by heavy equipment. Old ID's at well say Beardmore O & G is owner. No tank or other equipment found.

987564380 2013-03-19 Follow up to 1904828552. The valves from the 4 1/2 production casing to sales were open. The valves on the tubing were closed. The pumping unit had become over grown with vegetation and was not equipped with a power source. No identification posted at the well or tank. The empty 100 barrel steel tank was inside a dike that had roughly 3 of fluid across the bottom. Roughly a quart of oil had leaked from the bottom seal of the tank in to the dike and solidified across the bottom in dry weather. There was no sheen on the water. The water was field tested and found to have significantly less than 32 ppm Chloride content.
