Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
256342955 2009-09-24 The excavation around the wellhead is filled in. There is a swab rig on the well. The complaint is RESOLVED.
692163391 2009-04-28 I received a call on 4-27-2009 from Steve Collins (Massillon Fire Department, 330-833-1053) regarding an odor complaint at a well located at 106 Rolling Park Dr., Massillon, OH. On 4-28-2009, I met with John Casper (Rolling Hills Trailer Park) at the well site. A gas leak was reported on Sunday 4-26-2009 and the Massillon Fire Department responded. Petrox was notified and addressed the leak. During my inspection on 4-28-2009, I called Bill Magrell (330-565-5873) with Petrox to get the details of the incident. Gas had been leaking from under the production string casing hanger out of the surface casing. The surface casing was loaded with water to kill the gas. A hole was drilled in the surface casing to monitor for gas. During my inspection there was no gas present. Petrox will evaluate the leak and then repair the casing or plug the well. I informed the Massillon Fire Department of my findings. I talked to Cindy (Rolling Hills Trailer Park) later that day and informed her of the situation.
710964767 2009-05-05 The excavation at the wellhead is still open. There was no visible venting or odor coming from the port in the casing. I met with Justin (Cindy's son), who is in charge of maintenance.
738779186 2014-03-27 I was on location to obtain updated GPS coordinates for this well. During my inspection I found a plunger lift well with a 100bbl production tank and separator all enclosed in a fence. The well is located in a manufactured home park. The wellhead is in good conditioned has proper I.D. signage with 55psi of pressure on the tubing. There are no violations at this time.
