1219668431 2008-08-24 I witnessed the cementing of the 4 1/2 casing. There were no violations at this time.
1252520851 2009-09-09 Well is a pump jack well. Site has been graded and seeded. Residual from trees is stockpiled north side of the site. Stumps are stockpiled on site. Called Scott Wilkes, stumps need ground, removed or spread out so they will decay. Piles of stumps are not acceptable reclamation. Tank battery has two 110BBL tanks and one 1500 gal poly tank in it. Mechanical separator. Dike is in good repair. Spark arrestor has been installed. Permit 2061 is also produced into the tank battery.
1317869677 2013-03-25 The well is equipped with a pump jack. The well and tank battery are in good repair. The tank battery consists of two 100 BBL production tanks, a 50 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. The tank battery and lease road are being shared with permit #2-2061.
1625893914 2008-08-21 I performed a rig inspection. The pit liners and the location were in good shape. I found no violations at the time.
1820132285 2012-02-17 Well is a pump jack well. Drill area has been reclaimed. There is an electric line in conduit behind the well head, the conduit has parted at the connection box. The wires are intake but exposed. Called Randy and left message, have conduit repaired. There are three separators on site, separators need removed, they should not be stored on location. Tank battery is in good repair, two 100BBL prd tanks, one 1500 gallon poly tank and a mechincal separator in the battery. Dike is in good repair. Identification sign is posted. Spacing between the PRD tanks is 33 inches. Spacing between separator and PRD tanks is correct. Lease road is stable and constructed out of durable material.
-189279388 2008-08-24 I performed a rig inspection. The pit liners and the location were in good shape. I found no violations at this time.
-1961091949 2008-08-21 I witnessed the cementing of the 8 5/8 casing. There were no violations at this time.
-198450102 2012-08-16 My inspection today was due to a water well complaint at the Byler residence 2500ft from this well. My inspection found a pumping unit well in good repair. The 8.625 annulus was open. Review of well records showed nothing abnormal with casing and cementing program. The well is produced into the same tank battery as permit 22061. The battery has two 140bbl production tanks, one 1500 gallon poly tank and one mechincal separator. The dike is in good repair. I reviewed the plate and Access Geauga for property owner information. I contacted Scott Wilkes and enquired how royalties are being paid. He stated that he would have to review the plate, but royalties are paid to the landowners in proportionate amounts from the unit.
2079071517 2008-08-20 I performed a rig inspection. I GPS'd the well. The pit liners were in good shape as well as the location. I found no violations at this time.
