Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1139565360 2014-02-19 Well is equipped for production. There is a clamp on the 4.5 in casing to try and seal a leak. Gas can be smelled around well. Meter run is disconnected from the well. There is no chart in the meter box. Tank battery consists of 1 100 barrel tank. Accompanied by landowner Mr. Randy Wagner.
1660842060 2008-07-15 Inspection this day due to landowner complaint - well leaking gas. Checked and it is leaking quite a bit - called John Schneider and asked him to get it fixed in next day or 2.
-2038449084 2005-12-19 Inspection this day found well producing thru casing, current chart in meter, ID's pretty faded and peeling, no dike.
-790032331 2014-09-10 Performed an inspection on this well in response to a landowner complaint. Upon inspection, it was observed that this well is equipped for production, but is currently shut in. The entire location is overgrown with thick vegetation, indicating that no one has been to the well at least several months. There is a leak in the 4.5 casing above the ground surface that the operator attempted to seal off with a clamp. The well pressured up during the inspection and natural gas could be heard and smelled coming from the leaky casing. The well consists of 7 surface casing, 4.5 intermediate casing, and 2 tubing with a rabbit catcher. The tank battery consists of a 100 bbl production tank, a drip, and a meter run. The meter run is disconnected and is laying on the ground. There is no identification information posted at the well head or the tank battery. RBDMS production records indicate that this well produced 153 Mcf of gas in 2013. I will contact Mr. John Schneider of Stonebridge Operating and instruct him to put new ID's at the well and inquire about plans to put the well back in line.
