1172869331 | 2013-10-15 | I inspected this well to follow up on Chief's Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insuarnce as required by ORC 1509.07(A) and has been found in matierial and substatial violation for failing to maintain coverage. During my previous inspection on 8/1/2012 to follow up on a violation, I found that an ID sign had been posted on the tank and that the meter chart was changed on 6/12/2012. I issued a compliance notice for failure to report production for 2 consecutive reporting periods. My inspection today found that nothing has changed in the status of this well. The same meter chart, dated 6/12/2012 was still on the recorder box and had spun around numerous times, recording with the pen. The ID that was posted last year is tattered and the company phone number is not legible. The valves on the well and at the meter are open. This is a swab well and the chart indicates that the hole is loaded and the well requires swabbing to continue producing. This violation continues. | ||||

An inspection in 2007 found that there was a chart on the nearby meter which indicated that the well was producing natural gas. The operator has apparently failed to report this production as required. The well must be plugged and abandoned. Failure to comply with this Compliance Notice will result in additional corrective action up to and including bond forfeiture. | ||||

1632693638 | 2012-08-01 | FOLLOW UP TO COMPLIANCE NOTICE # 866351948. During this follow up inspection, I found that proper ID signage had been posted on the tank battery. The previous report had indicated that production had not been reported since 2002 and a check of RBDMS indicated that the last production was reported in 2009. A check of the meter chart indicated that the well was producing and the chart was changed on 6/12/12. If this well is producing as the meter chart would indicate, then production for 2010 and 2011 needs to be reported as required. | ||||
-1783699504 | 2007-10-29 | The Uniontown office received a complaint from Gerald Turck concerning the production and revenue dispersment from this well. I inspected the site and found that the well was in production based on the chart response at the meter. The last reported production from this well was in 2002. However, Mr. Turck showed me his last revenue check which showed production of 805 MCF from March 2004 through July 2004. He claims that the well has been in production and that Svetlak has not been paying royalty owners. I explained to Mr. Turck that the Division does not get involved in legal issues between the landowner and the operator. I requested a copy of his last statement to investigate if Svetlak has been paying severance taxes. | ||||
