439941334 2013-03-11 Production well inspection, this well consists of 1-100 barrel tank and a pumping unit. There is no legible identification and the stuffing box needs repaired. A tributary of Monday creek if flowing 12' from the dike, it is starting to erode the dike.
442571381 2009-05-21 An inspection revealed unlawful venting of gas without the use of a flare and missing identification. It was also noted that the earthen dike placed around the 100 bbl tank did not contain a front wall for containment. A tributary of Monday Creek, Lost Run, was observed behind the tank location approximately 100 feet away. A second 100 bbl tank was observed near well location and found not connected to the well operation. The tank is located approximately 75 feet away from first tank. The valve on the bottom of this tank (load line) was found open and showed signs of leakage as approx 2' x 2' area of oil was observed. The tank was empty at the time of the inspection. Plastic lines found connected from well head, 4.5 casing were traced to area in the woods and within close proximity to the tributary. At the time of the inspection, the valve was closed at well head and lines were found cut. There were no connections observed from the tank to this location or routed to the well for annular disposal. Note: The well is located on property that had been mined for coal. Records indicate that no brine has been reported for this well from 1993 to 2008.
