1330487458 2012-02-24 During my inspection I completed a final restoration status check and found leftover oil field debris at the location. It consisted of three pieces of 4.5 casing, a 55 gallon plastic drum of crude oil laying on its side, three electric poles and a strand of wire, a 3 metal pipe near the tank battery, and a 2 metal line buried beneath the lease road. The tank battery also needs to be addressed as there is gravel where the tanks sat and the dike needs removed and the area graded and seeded. Final restoration not finished.

-1768151535 2006-10-05 On site by request of Dave Crow. Follow up on report of violation. No identification posted at the well head or the storage site. Does not appear to have been produce for some time. Well and pumping unit are over grown w/brush and trees. Pulies on pumping unit motor and gear box are rusty indicating none use.

Storage site shows signs of brine spills around tank. Tank appears to be leaking around the bottom. I would recomend not using the tank untill repaired, or remove it and install a good tank. Called the phone number that is in RBDMS not a working number.

-869011398 2012-01-19 A Final Restoration inspection was requested by well owner, Mr. Bill Stachler (sp?). During the inspection, it was noted that restoration does not meet ORC 1509.072. Oilfield debris consisting of plastic and steel line, electric line and pole, joints of steel casing tanks may have sat on, and section of load line. Mr. Stachler was present for the inspection and agreed to remove all debris.
