-1753825869 2012-07-30 On 7/30/2012, I conducted an urban production well inspection status check. My inspection found the well was a plunger lift unit surrounded by a chain link fence with no lock that was in good condition. The tank battery was surrounded by a chain link fence with no lock and contained one 100 barrel oil storage tank, one 50 barrel brine storage tank, a separator, and the proper identification was posted. I noticed no visible spills or leaks. The lease road was the parking lot across from the Truck World gas station. I called Energy Resouces America at 330-953-1813 and asked to have locks placed on both gates to the tank battery and well head.

577957462 2014-07-08 I was on location for a status check. I found during my inspection the well in production with a plunger lift unit. The tank battery consisted of a 100 barrel production tank, a 50 barrel, poly brine tank and a separator. There was a locked, chain link fence surrounding the well and tank battery. A dike surrounded the production equipment. Identification signs were posted. Safety equipment was installed on the production tank. The lease road was a paved parking lot for a truck stop.
