-103889557 2011-07-31 I witnessed the cementing of the surface casing. The cement was verified with Phenolphthalein solution, 1%. Good returns were observed and cement stayed at the surface. Both the surface and conductor hole were drilled on fluid. Note: Circulation lost prior to surface job at approx 250'. Returned to the location later this evening and witnessed the BOP test. The BOP passsed the required test. Compliance Notice
1091468302 2014-06-03 Status check inspection. This well was drilled in 2011 in a urban area. The fencing around the tank battery and pumping unit has been started but not finished. There are no locks and there no access road. I have contacted P.E.P. drilling supervisor Mark Smith by telephone. I left him a message at 1400 hours.

-205704633 2014-08-04 I called Mark Smith with PEP drilling. I informed Mark of the need to plug this well. They have a valid plug permit. Mark said they are moving a rig in two weeks to plug this well.
