2155i90 2002-05-28 Met with Dan Cook (330-753-5081) with B & C Machine regarding the current status and future plans for this well. Mr. Cook indicated that the well does occasionally provide natural gas to the facility and is therefore in production. Advised Mr. Cook that the well lacked proper signage and spill containment around the tank battery. Also provided Mr. Cook with a copy of the Plugging Company list for future reference. Will check the site in several weeks to verify that the sign and dike were installed.

-937391432 2005-08-03 Met on site with Steve Hagen (Intrepid Well Service) to determine annular flow from this well prior to plugging. The well is located on the north side of the PPG Area. Annular flow, at approximately 2 gpm was present. PPG plugging requirements will be necessary.
