Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1301666873 2011-04-01 Requested by Rob Stonerock to investigate well owned by Triad Resources Inc. Well is idle and no production equipment. It is swedged in on 4.5 casing. New home and garage 50 ' south of well. The address is 172 SR 162 Sharon Center. This well has not produced for years. No production has been reported. Well needs plugged due to being idle and housing development surrounding location.
-1935791631 2011-06-22 Rocky with Triad Resources requested this inspection and names of anyone that is purchasing or may be interested in purchasing this well. The inspection found that all equipment has been removed and the only thing remaining is 4-1/2 casing with a swedge. I advised Rocky that without salvagable equipment, Triad might just as well plug and abandon the well.
-1976138141 2014-02-27 Visited location on 02/27/2014 to set bottom plug. Set bottom plug from 3,610'-3,210' through tubing. Plan forward is to shut down for the weekend and resume operations Monday.
-664041033 2011-05-06 No response to compliance notice. Well remains idle. Sun, 50 degrees.
-840499383 2014-03-03 Visited location on 03/03/2014 to finish plugging out the well. Ripped 4 1/2 casing at 2,490' and pulled casing from this point. Set Big Lime plug from 2,296'-1,996', Berea plug from 514'-310', and surface plug from 150'-surface. All plug jobs went well, and good cement was brought to surface. Plan forward is to cut well off 3' below ground level, weld on a steel plate, and cover with soil.
