Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1081248333 2004-04-05 No violations noted on the day of my inspection tank site is shared with adjacent well.
1409085501 2014-08-26 I was on location to witness Formation Cement Company set a bottom hole plug through 1.5 tubing in the 2 7/8 casing from 3802-4165' using 10 sacks of Class A cement.
1409173505 2014-08-27 Operator tagged the top of the bottom hole plug at 3263'. I was on location to witness Timco Wireline services shoot the 2 7/8 production casing at 3165', 2970', 2750', and 2506' before the pipe was free from the hole. Formation Cement then set a 300' plug from 2200-2500' using 21 sacks of Class A cement. They then pulled 14 joints when the hole unloaded oil and gas. Operator was able to shut the well in by pumping fresh water down the annulus. They will sit on the well overnight and return tomorrow to finish plugging the well.
1409241355 2014-08-28 Operator tagged last plug at 2300'. Cemenco pumped 140 bbls down the backside and did not see circulation. I was on location to witness Cemenco Service set a Berea plug from 1240-1540' using 25 sacks of Class A cement. Operator then pulled 7 joints and the hole started making oil and gas. After it died down, they pulled 8 more joints and Cemenco pumped another 35 bbls with no circulation. They then set another plug from 825-1075' using 20 sacks of Class A cement in hopes of plugging off old 4.5 Berea perfs. Operator then pulled 20 more joints. The well did not load back up with oil and gas. They will sit on plug over the weekend and tag the top of cement on Tuesday.
1409688809 2014-09-02 Operator tagged the top of the Berea plug at 1376' and then pumped on hole until they established circulation. The well circulated oil and water. Cemenco then set another plug from 1240-1376' to cover the Berea. Operator pulled all of the tubing from the hole. Timco Wireline Services then ran a cement bond log and had the top of cement on the 4.5 at 1050'. Timco then shot the 4.5 at 1020' then at 720'. Operator pulled the 4.5 up to 363' in order to set a plug from 363' to surface. Cemenco pumped the hole clean of oil and set the plug. There was good circulation of cement at the surface. The 4.5 locked up as they were pulling it out of hole and operator was only able to pull 2 of the 11 joints of casing, thus leaving 9 joints in the hole. Cement was 10' down the backside and around 25' down the 4.5 from surface. Operator will let cement set overnight and come back to cut off casing and wellhead. They will grout down the 4.5 and backside.
