Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1248921391 2009-07-27 Capstar Drilling Rig 4 is moving in and will spud the well tonight. The shale pit has been dug and lined. The blow pit will be dug later today.
-136411948 2009-08-13 The drilling pits have been closed and the tank battery is being built. No violations at this time.
1392981310 2009-08-06 Transcontinental was on site to start solidifying the pits. The dump truck application was much more effective in keeping dust to a minimum.
1445026252 2009-09-03 The tank battery is being built. The site has been rough graded, the pits have been solidified and covered. The well has not been fraced.
-178342429 2009-08-04 The well is completed and Capstar has started rigging down. They are scheduled to move off location on 8/5. The pits are fenced, lined and in good condition.
-1835100231 2009-10-08 The tank battery has been partially completed and the well is scheduled to be fraced next week. No violations at this time.
183710282 2010-06-21 Preliminary reastoration is past due. The affected area has been graded but needs to be seeded and mulched. I met with Cal Marks and discussed both this well and the Hutcheson well which is also past due.
-1921536654 2010-07-23 Preliminary restoration is approved.
33943123 2010-06-02 The well is in production and the tank battery and wellhead are fenced. Preliminary restoration is due now. The site still needs some grading before seed and mulch. If the work is not completed by the end of June, a NOV will be issued.
-520415029 2010-01-06 The well is now in production. Preliminary restoration will be completed as soon as the weather permits in the spring. No violations at this time.
766915063 2009-10-21 This status check inspection found that the well has been fraced and tubed. The top perforation was reported to be at 3722' and the producing interval is less than 4000' deep. The spacing and set back minimums are therefore based on 20 acre requirements. The tank battery is nearly complete and the well should be ready to turn on in the near future. Preliminary restoration is due by the first of May 2010.
-801164179 2009-08-07 Met on site with Cal Marks. Problems and complaints from two neighboring properties to the south and west continue. I suggested that he notify Macedonia police and fire before he does anything that the neighbors might notice or moves equipment on site including the frac equipment and the service rig.
-818527446 2009-07-31 Meet with surveyor to confirm the as-drilled location of the well. The location issue was raised and challenged by a neighbor. This physical inspection confirmed that the well is being drilling in the correct and permitted location.
